The importance of early diagnosis and the implementation of multimodal and personalized interventions

This paper is based on the presentation of a clinical case involving a girl attending the second grade of primary school, with a history of previous language difficulties. The first neuropsychological evaluation, carried out in March 2021 by Dr. Daniela Gallo, highlights the presence of a Expressive Language Disorder (F80.1) still active, with significant repercussions on learning to read and write, such as to configure a high risk of developing a Specific Learning Disorder in the field of reading and writing (Developmental Dyslexia F81.0 e Developmental Dysorthography F81.1).

ROAR, an innovative reading skills screening tool

I Specific Learning Disorders (SLD) They affect 5-15% of the global population (APA, 2013), with dyslexia being one of the most commonly diagnosed disorders (MIUR, 2024). They can have significant repercussions on school adjustment and psychological well-being, influencing not only performance but also the emotional health of individuals (Scorza et al., 2018a).
This thesis aims to contribute to the ongoing Italian validation process of the instrument. The results of the descriptive analyses and linear regressions were obtained by comparing two groups, consisting of a total of 13 children, with different levels of decoding ability.

Observation of WISC-IV indices (IRP, IML and IVE) in a sample of subjects

One of the erroneous beliefs is that poor performance at school is always linked to an intellectual deficit, instead there is a large category of intelligent children/teens who suddenly encounter a barrier compared to their peers: what is simple for others is complicated for them.
On the basis of the literature and taking into consideration the studies that relate the WISC-IV to Specific Learning Disorders, with this thesis contribution we aimed to observe the cognitive profiles outlined by the WISC-IV scale in a reference sample extracted from the patients of the SOS Dyslexia center of Casalecchio di Reno (BO).
Specifically, we focused on the observation of the values of the specific IRP, IML and IVE indices (whose discrepancy can provide us with information on the basic cognitive processes of DSA) in a clinical sample with well-defined diagnoses; distinguishing the subtypes in the cases of DSA diagnosis based on the scholastic difficulties identified in the diagnostic process. We tried to understand, through the observation of the trend of these indices, whether it was possible to identify profiles that may be typical for subjects with DSA by formulating hypotheses on any prevalent profiles in the selected sample, in particular looking at the relationship between non-verbal logical functions, working memory and processing speed.

Bilinguals and Reading Comprehension

Reading Comprehension in Bilingual subjects seems to be less efficient than in Monolingual peers, with consequences that lead to early school dropout. The literature offers an important contribution regarding the Reading Comprehension of Monolinguals, which would be influenced by the Executive Functions and Working Memory of the subjects. As regards Bilinguals, the literature looks more at a linguistic component as a factor underlying Reading Comprehension. The aim of this meta-analysis was to identify which functions are mostly responsible for the Reading Comprehension performance of Bilingual subjects. For this purpose, 13 articles were selected that compared the Reading Comprehension performance of Monolingual and Bilingual subjects, coding in addition to the Reading Comprehension variable, two reading variables and one vocabulary variable. The following were calculated: effect size of each variable to measure the difference between Monolinguals and Bilinguals. The results of our meta-analysis highlight that reading skills (both lexical and phonological) do not seem to influence the performance in Reading Comprehension of Bilingual subjects. Subsequently, a meta-regression was conducted to verify the presence of an influence of vocabulary on Reading Comprehension tasks. Our results show that the size of the L2 Vocabulary of Bilingual subjects influences their performance in Reading Comprehension.

Reading and text comprehension disorder

There is extreme heterogeneity and neurodiversity in children with dyslexia, dictated by the social context, by the support and requests made to the child and at the same time by his personal characteristics and his perception of difficulties and strengths. The "evolutionary" nature of the disorder should not be forgotten, in fact DSA tend to improve spontaneously over time, so the most serious moment of the disorder coincides with the greatest demands on the reading and writing level, the school period; specifically, in reading, a tendency to improve more has been seen in medium-mild dyslexics, but the disorder seems to never fully compensate. In addition to the degree of severity, the specific learning disorder changes shape over time, think of the countless cases of learning disorder with a previous language disorder. The paper delves into the specific reading disorder with particular attention to text comprehension, which should not be considered as an aspect disconnected from learning, but a fundamental aspect in the construction of the child's school life.

Analysis of DSA with Machine Learning techniques

In this thesis, a research is illustrated that involved 40 secondary school children who were diagnosed with DSA. The data from the tests administered were correlated with each other and analyzed with modern machine learning techniques. We identified a direct correlation between the IRP Perceptual Reasoning Index and the Trog-2.
Regarding reading, in the dyslexic subjects of our sample there is a more evident (and lasting) deficit in reading speed, when compared with correctness.
With regard to the writing tests, carried out in normal conditions and in conditions of articulatory suppression, it emerges that writing performance worsens considerably if carried out in conditions of articulatory suppression, both for the correctness aspect (word dictation) and for the speed aspect. (writing numbers in letters).

From discomfort to psychological disorder in subjects with Specific Learning Disorders

This thesis is dedicated to the in-depth analysis of the psychological aspects that are often underestimated and taken little into consideration within an already complex picture in itself which is characteristic of subjects with Specific Learning Disorders.
To better understand its complexity, it is important to use an integrated approach that correlates all aspects of the disorder, thus not neglecting the emotional, psychological, neuropsychological and relational aspects that characterize it.
A broader view above all allows clinicians to initiate more in-depth evaluation, diagnostic and intervention paths with the ultimate aim of ensuring the psychological well-being of the individual.

We have therefore tried to deepen the research in this area, considering that when we talk about "reading comprehension" we are not referring to a single ability, but rather to a series of abilities, with as many underlying cognitive functions, which, working in synergy, allow for effective reading and an equally effective extrapolation of important information from what has been read.

Non-Verbal Learning Disorder: when difficulties do not reside only in language

In this research, some historical notes will be initially shown to outline the path that led to the discovery of Non-Verbal Learning Disorder (NLD) and its developments up to the present day. Subsequently, the common etiological hypotheses of the disorder will be briefly outlined, to indicate what research to date has indicated as the most frequent and accredited causes of the manifestation of NLD. Continuing, ample space will be dedicated to the explanation of the criteria listed in the guidelines for formulating the diagnosis, with an overview of the tools widely used and recommended for the evaluation of children and adolescents with suspected NLD, indicating the expected results for the profile of the disorder in question.

DSA is characterized by an extreme heterogeneity in clinical pictures, both with respect to the different dimensions of the disability and with respect to its long-term evolution (Ruggerini et al., 2004).
The presence of a learning disorder can be a circumstance that is accompanied by great emotional suffering in childhood and, in this sense, can contribute to determining a pathological deviation of development and, considering the high degree of comorbidity detected between DSA and psychopathology , whether it is read as a co-occurrence or as a consequence, it is right that the clinician who is responsible for establishing the diagnosis and therapeutic plan of a patient with DSA questions whether or not it is appropriate to combine neuropsychological rehabilitation /patient's speech therapy and psychotherapy (Luci and Ruggerini, 2010).
In this work we will therefore examine the relationship between Specific Learning Disorders and Psychopathology in developmental age, starting from the comorbidity between the two.

Evolution of Specific Learning Disorder between neuropsychological characteristics and metacognitive aspects

In the present research, two clinical cases were examined with the aim of following the evolution of DSA over time, from the moment of diagnosis to early adulthood, with particular attention to the cognitive profile and the psychological aspects mediated by metacognition. What emerges from the clinical analysis confirms a permanence of specific difficulties and neuropsychological deficits, especially with regard to the functioning of working memory, information processing speed, inhibitory control and attentional and executive functions. In metacognitive terms, these characteristics are frequently associated with a good awareness of the difficulties associated with the specific learning disorder, not only in school but also in daily life.
